Complete Rehab & Sport is a participating provider in most major insurance companies, including Medicare and No Fault. Please call the office to check participation status of your insurance by phoning 631.654.5282, and our expert staff will attempt to verify your benefits and notify you of the options.

In most cases, a prescription for physical therapy treatment written by your physician, physician's assistant, nurse practitioner or dentist is required in order to utilize your coverage benefit. However, it is not a necessity for some plans and direct access to our services is often an option.

Forms and Patient Information

Please click the buttons below to download and complete the following forms in preparation for your initial visit.  The “Initial Forms Packet” which includes medical history, pain scale, advanced directive, co-payment agreement, confidentiality notice, and signature on file, then choose one of the three “assignment of benefit” forms and the “functional questionnaire” that appropriately corresponds to your situation.  This will help make your initial experience with us as efficient as possible, allowing us to remain focused upon "What we can do to help you."
